You can either bring your Bank Account details to the Studio and complete an application form there or even easier please click the PDF application form link below, download and complete per instructions below.

After downloading the PDF you can open it online and completed the following Sections: (or you can print it and manually fill it in using a black pen)

• YOUR DETAILS
• DEBIT ARRANGEMENT
• CHOOSE YOUR PAYMENT METHOD

Please then print, sign & date.  Then scan or photograph the signed form and return it to us via email [email protected] or give it to the teacher at your next class.

Yogita Yoga will then set up your account for future membership / contract to be covered by this contract.
